NEUROPSYCHOLOGICAL ASSESSMENT OF COGNITIVE IMPAIRMENT IN PARKINSONISM: CURRENT METHODS, SCALES, AND DIAGNOSTIC SIGNIFICANCE

Abstract

Cognitive impairment is one of the most significant non-motor manifestations of Parkinson’s disease and other parkinsonian syndromes. Its early detection is complicated by the heterogeneity of the neuropsychological profile and by the influence of motor, emotional, and somatic factors on cognitive test performance.

The aim of this article is to systematize current approaches to the neuropsychological assessment of cognitive impairment in parkinsonism. The diagnostic utility of cognitive screening instruments and comprehensive neuropsy­chological assessment is reviewed, including the Montreal Cognitive Assessment (MoCA), Mini-Mental State Exami­nation (MMSE), Stroop Test, verbal fluency tests, and methods for assessing memory and visuospatial functions.

Particular attention is given to the principles of diagnosing mild cognitive impairment in Parkinson’s disease according to the criteria of the Movement Disorder Society. The available evidence indicates that reliance on a single global cognitive score is insufficient for a comprehensive characterization of a patient’s neuropsychological status.

A comprehensive approach that enables the assessment of multiple cognitive domains while taking into account the effects of motor, emotional, and behavioral factors appears to be the most informative strategy.
