The authors report that a retrospective analysis of surgical management was conducted in 85 patients with perforated gastroduodenal ulcers (PGDUs) who received treatment between 2020 and 2022. The study examined patients’ demographic characteristics, timing of hospital admission, ulcer location and size, the nature of peritonitis, associated complications of peptic ulcer disease, comorbidities, and the distribution of surgical procedures performed. Duodenal ulcer localization was identified in 82.4% of cases, whereas gastric ulcer localization accounted for 17.6%. The most frequently performed procedure was conventional suturing of the perforation, accounting for 58.8% of cases. Excision of the duodenal ulcer combined with duodenoplasty was performed in 22.4% of cases, while resection procedures accounted for 9.4%.
The authors conclude that the conventional approach did not always involve a comprehensive assessment of the size and location of the ulcerative infiltrate, the condition of the pyloric sphincter, the extent of cicatricial changes, and the nature of peritonitis. The findings support the need for a differentiated approach to selecting the extent of surgery in patients with perforated gastroduodenal ulcers.
The aim of the study. To improve the outcomes of surgical treatment in patients with perforated gastroduodenal ulcers by analyzing the results of conventional approaches and optimizing surgical management strategies.
Material and methods of the study. The study was conducted at the clinical base of the Department of Surgical Diseases and the Andijan Branch of the Republican Scientific Center for Emergency Medical Care over the period from 2020 to 2025. To address the objectives of the study, a total of 178 patients who underwent surgery for perforated gastroduodenal ulcers (PGDU) were examined. In accordance with the aim and objectives of the study, the patients were divided into two groups:
Results In the comparison group, during the period from 2020 to 2022, surgical treatment of patients with PGDU was performed according to conventional, generally accepted approaches, including a standard set of diagnostic procedures and preoperative preparation measures. Various surgical procedures were performed, with closure of the perforation being the most common intervention.
