Background and importance. Cerebral aneurysms pose a huge danger to the patient’s life, since they have a high risk of rupture. In the overwhelming majority of observations, aneurysms are located in the anterior part of the arterial circle of the cerebrum (circle of Willis) and only 3% of aneurysms are localized in the vertebrobasilar vascular system.
Clinical presentation. In this article, we describe a case of successful cross-sectional revascularization of a massive aneurysm in the M1 segment of the left middle cerebral artery (MCA) using a combination of microsurgical revascularization of the MCA with extracranial microvascular anastomosis and complete aneurysm resection with intracranial microanastomosis. Radical treatment of the aneurysm was performed on the basis of revascularization of the left MCA territory.
Conclusion. The chosen surgical technique prevented the development of brain edema and compression, recurrent subarachnoid hemorrhage and re-rupture of the aneurysm. It is concluded that this method of treatment is effective.
